Brief Description : The Portsmouth Rehabilitation and Reablement Team are looking for an apprentice to join their team, you will deliver a person centred customer service alongside working to gain a Level 2 Adult Care Worker Qualification.

 

Qualifications Required : Good understanding of English and Maths.
Maths and English will be assessed as part of the interview process.

Working with the Portsmouth Rehabilitation and Reablement Team, you will deliver a person centred customer service whilst working towards a Level 2 Apprenticeship in Adult Health & Social Care.

The Job role/Duties
Portsmouth Rehabilitation and Reablement Team (PRRT) is a
multi-disciplinary team with staff employed by Health and Adult
Social Care for residents of the Portsmouth City area. The team visit clients in their own home to provide short term support with a view to promoting independence. The team may support those who are overcoming a stay in hospital by providing immediate assessment on discharge and also work to prevent any unnecessary hospital admission by providing timely support in the home environment. Our team is made up of a range of disciplines including Physiotherapy, Nursing, Social Work and Occupational therapy, alongside our Rehabilitation Assistants who deliver personalised rehabilitation programmes which can include exercise programmes, support with personal care (e.g. washing and dressing), meal preparation and support with medication. Our team aims to support clients to reach independence goals which matter to them. Clients receiving our support may have experienced a short term illness or accident (e.g. fall) and may need some support to regain their confidence and strength. At the end of the clients journey with our team, we can assess for any long term needs which may have been identified.

What you will achieve

12 months experience of working in the Adult Health & Social Care Services.
Level 2 Diploma in Care (RQF). This qualification, promoted and valued by employers, is achieved by a combination of direct teaching and self-directed learning.

Working Week : 37 hours per week between 7am-10pm You will be required to undertake some work during the evening an
Salary : £300.00 Per Week
